Recently, Tangshan People's Hospital admitted a patient with knee pain. The patient had experienced bilateral knee pain without any obvious cause six years prior, making movement difficult. He had previously received acupuncture treatment at a local clinic with limited success. He then consulted at Tangshan Second Hospital, where examinations and X-rays led to a diagnosis of bilateral knee arthritis, and he was advised to undergo joint replacement surgery (specific treatment details unknown). Description of the condition
The patient is a 56-year-old female.
Chief complaint
I have had pain in both knees for six years and have been unable to move them. Recently, the pain in my left knee has worsened.
Present Illness
The patient developed bilateral knee pain six years ago without any obvious cause, making it difficult for him to move. He had previously received acupuncture and other treatments at a local health center, but the effects were unsatisfactory. On August 19, 2022, he visited the Second Hospital of Tangshan City. After examination and X-rays, he was diagnosed with bilateral knee osteoarthritis and was advised to undergo joint replacement surgery (specific treatment details unknown). Recently, the pain in his left knee has worsened, and for further diagnosis and treatment, he visited our hospital today. After examination and review of X-rays, he was admitted to the hospital with the same diagnosis.
Since the onset of the illness, the patient has been in good spirits, sleeping well, with a normal appetite, normal bowel movements, and no change in weight.
Specialist examination
There were no obvious deformities in either knee joint, and no obvious redness, swelling, or ulceration of the local skin. Both knee joints exhibited clicking sounds but no locking. The patellar tap test, patellar compression test, and patellar grinding test were positive, while the medial and lateral stress tests were negative. There was tenderness in the medial joint space and no tenderness in the lateral joint space. The Lachman test was negative. Left knee extension was approximately 20°, and knee flexion was approximately 100°; right knee extension was approximately 10°, and knee flexion was approximately 100°. Both hip and ankle joints had normal range of motion. Local skin temperature and sensation were normal, with good dorsalis pedis pulse and peripheral circulation.
Auxiliary examination: DR (Tangshan Second Hospital, 2022-8-19): Osteophytes in both knee joints, narrowing of the medial joint space.
